What is the most accurate screening method for Down syndrome?

The most accurate method of testing is called NIPT (Non-invasive prenatal testing), which has an accuracy of up to 99.9% compared to amniocentesis and does not risk causing miscarriage.

How many types of Down syndrome screening are there? What are they?

NIPs or NIPT – is a test for fragments of fetal DNA that have passed from the placenta and circulate in the mother’s blood. It is recommended to test after 10 weeks of pregnancy onwards. This method is the most accurate screening method, up to 99.9%.
First trimester screening – includes the nuchal translucency (NT) test, pregnancy-associated plasma protein A (PAPP-A) test, and hCG hormone test. It is performed between 11 and 14 weeks of pregnancy (approximately the third month of pregnancy). The accuracy is approximately 90%.
Second trimester test , also known as Quadruple test – tests for 3 important substances: 1) alpha feto-protein (AFP), 2) chorionic gonadotropin (hCG), and 3) unconjugated estriol (uE3). Or if testing with 4 important substances, the inhibin A test will be added to reduce errors in the test. It is recommended to test at 15 – 20 weeks of pregnancy (approximately the 4th month of pregnancy). Accuracy is approximately 80%.
Staged testing – This includes testing in the first and second trimesters, with timed tests to screen for fetal abnormalities. In addition to Down syndrome, there are also neuropathy and other genetic diseases. In Thailand, this method is not popular because the cost is as high as NIPT, but the accuracy is not as good, and the mother has to come to the hospital multiple times.

What is Down syndrome?

Down syndrome is a common genetic disorder caused by an extra copy of chromosome 21 (total of three copies). This abnormality causes children to be born with abnormalities in physical and intellectual development. It is the most common cause of learning in the world related to chromosomes. The average chance of this condition is 1 in 700, but the chance of this condition increases as the mother gets older. For those over 35, the chance of this condition is as high as 1 in 250.
