ผู้ยื่นคำขอต้องเป็นผู้รับผิดชอบในความถูกต้องแท้จริงและความชอบด้วยกฎหมายของสิ่งส่งตรวจที่นำมาสำหรับการตรวจพิสูจน์ดีเอ็นเอ

การตรวจพิสูจน์นี้จะรับผิดชอบเฉพาะสิ่งส่งตรวจที่ผู้ยื่นคำขอส่งมาเท่านั้น สำหรับกรณีที่สิ่งส่งตรวจไม่ได้รับการยืนยันตัวตนโดยเจ้าหน้าที่อย่างเป็นทางการ บริษัทตรวจจะออกเพียง “ความเห็นเชิงคำแนะนำ (Advisory Opinion)” เท่านั้น

บริษัทตรวจจะเก็บรักษาข้อมูลส่วนบุคคลของผู้ยื่นคำขอไว้เป็นความลับ

ก่อนเริ่มทำการตรวจ บริษัทจะทำการประเมินเบื้องต้นของสิ่งส่งตรวจ หากพบว่าสิ่งส่งตรวจไม่เป็นไปตามข้อกำหนด ห้องปฏิบัติการจะขอให้เก็บสิ่งส่งตรวจใหม่อีกครั้ง หากผู้ยื่นคำขอไม่สามารถจัดส่งตัวอย่างใหม่ได้ การว่าจ้างจะสิ้นสุดลง โดยบริษัทจะคืนค่าตรวจร้อยละ 80 ของจำนวนเงินทั้งหมดหลังหักค่าใช้จ่ายเบื้องต้นที่เกิดขึ้นจริงในการดำเนินการตรวจ

ค่าใช้จ่ายเบื้องต้นที่เกิดขึ้นจริงในประเทศไทยประมาณ 7,600 บาท ประกอบด้วย ค่าอุปกรณ์เจาะเลือด/เก็บ DNA 500 บาท, ค่าบริการเจาะเลือด / เก็บ DNA ท่านละ 800 บาทจำนวน 2 ท่าน ค่าขนส่งเลือดไปยังประเทศฮ่องกงและขั้นตอนศุลกากร 5,500 บาท

อาจมีค่าบริการจากทางฮ่องกง กรณีมีการใช้น้ำยาไปในการตรวจแล้ว ซึ่งทางฮ่องกงจะเป็นผู้แจ้งจำนวนเงินมาอีกครั้ง

เมื่อเริ่มกระบวนการตรวจแล้ว หากไม่สามารถแปลผลได้เนื่องจากมีปริมาณดีเอ็นเอไม่เพียงพอ ปริมาณดีเอ็นเอของทารกในครรภ์ต่ำ สิ่งส่งตรวจปนเปื้อน เกิดภาวะเม็ดเลือดแดงแตกจากการขนส่งที่ไม่เหมาะสม หรือสิ่งส่งตรวจปนกันกับของบุคคลอื่นอันเกิดจากเหตุของผู้ยื่นคำขอเอง บริษัทมีสิทธิ์ที่จะขอเก็บสิ่งส่งตรวจใหม่และปรับระยะเวลาการตรวจให้เหมาะสม หากผู้ยื่นคำขอปฏิเสธหรือไม่สามารถส่งตัวอย่างใหม่ได้ การว่าจ้างจะสิ้นสุดลง โดยบริษัทจะคืนค่าตรวจร้อยละ 50 ของจำนวนเงินทั้งหมดหลังหักค่าใช้จ่ายเบื้องต้นที่เกิดขึ้นจริงในการดำเนินการตรวจ

รายงานผลการตรวจพิสูจน์ความเป็นบิดาที่ออกโดยบริษัท เป็นผลที่ได้ภายใต้เงื่อนไขที่ไม่มีสิ่งรบกวนจากภายนอก และไม่อยู่ในกรณีการตั้งครรภ์แฝดหรือบุคคลที่มีความสัมพันธ์ทางสายเลือดใกล้ชิด

รายงานผลการตรวจที่ออกโดยบริษัทเป็นเพียงการบรรยายข้อเท็จจริงตามหลักวิทยาศาสตร์และเทคโนโลยีที่มีอยู่ในปัจจุบัน ไม่ถือเป็นการรับรองสถานะทางกฎหมายใด ๆ
ข้าพเจ้าได้อ่าน หรือได้รับการอ่านหนังสือแสดงความยินยอมเกี่ยวกับการตรวจดีเอ็นเอเพื่อพิสูจน์ความเป็นบิดาแบบไม่รุกราน (Non-Invasive Prenatal Paternity Test) ข้างต้น และยินยอมรับเงื่อนไขทั้งหมดโดยสมัครใจ
Informed consent for Non-Invasive Prenatal Paternity Test (NIPPT)
- Applicant should be responsible for the authenticity and legality of the samples provided for DNA
- The identification is only responsible for the samples submitted by the applicant, for samples provided by applicant without any official identification, the test company only issues the ‘Advisory Opinion’.
- The test company shall keep confidential personal information for the applicant
- A preliminary judgment would be made by the test company on the sample before the test begins, the laboratory will request re-collect sample if the sample is not meet the requirement. If the applicant is not able to submit second sample, the entrustment will be terminated while the test company shall refund 80% of the testing fee to the applicant withhold the required upfront cost of identification.
- The actual upfront cost of identification in Thailand is approximately 7,600 THB, comprising: 500 THB for blood collection/DNA sample equipment, 800 THB per person for two people for blood collection/DNA sample service, and 5,500 THB for transporting blood to Hong Kong and customs clearance.
- There may be additional service fees from Hong Kong if reagents are used in the testing, which will be determined by Hong Kong separately.
- Once the test starts, if the lab is unable to interpret the results of the test due to not enough DNA in samples, low fetal fraction, sample contamination, hemolysis caused by improper transport or mixing with other people’s samples caused by the applicant’s own reasons, the test company has the right to recollect a new sample and update the test period. If the applicant refuses or fails to submit the sample again, the entrustment will be terminated while the test company shall refund 50% of the testing fee to the applicant withhold the required upfront cost of identification.
- The actual upfront cost of identification in Thailand is approximately 7,600 THB, comprising: 500 THB for blood collection/DNA sample equipment, 800 THB per person for two people for blood collection/DNA sample service, and 5,500 THB for transporting blood to Hong Kong and customs clearance.

- In the event that results cannot be obtained on the second attempt: The applicant has the right to ask for a full refund after deducting the actual initial costs incurred in the testing process.
- The paternity identification reports issued by the test company are obtained on the premise of excluding exogenous disturbance, multiple births and close relatives.
- The identification reports issued by the test company are only a factual description made under the existing scientific and technical conditions and cannot represent the acceptance of any legal status.
Applicant have read or have had read the above informed consent information about the test company Non-Invasive Prenatal Paternity Test and willingly accept all above terms.
